Age must be 18 to 100. Sleep from above 0 to under 16 hours, training 0-7 days, smoking only 0 or 1, BMI 12 to 60. Outside that range the model will not run.
Here you get model years. BMI separately computes weight from height. The habit scale stays on a 0-100 index and does not call the result biological age. BMR counts kilocalories, not years.
Sleep under 6 h adds 2, from 6 to under 7 adds 0.5, 7–9 adds 0, over 9 adds 0.5. Zero training days add 2, 1–2 add 0.5, 3–5 add 0, 6–7 subtract 0.5. Smoking adds 10. BMI under 18.5 adds 1, 18.5–25 adds 0, over 25 to under 30 adds 1, 30+ adds 3. Alcohol up to 7 units adds 0, 8–14 adds 0.5, 15+ adds 2. A unit is a sketch: about 330 ml of beer or 100 ml of wine, not a legal definition.
Index = 70 − offset × 5, clipped to 0-100. At offset 0 you keep 70. At +19 it drops to 0. The model result has one decimal place.

model_age = age + offset. offset = sleep_off + training_off + smoking_off + BMI_off + alcohol_off. index = min(100, max(0, 70 − offset × 5)). You add those offsets, not the raw sleep hours or the raw BMI number.
